To refer, please click on the link below and make your referral via our website. Referral Form – Oakleaf Enterprise (oakleaf-enterprise.org) Once we have received your referral, one of the team will get in touch to complete your registration, after which you will be able to participate in your chosen activity on Zoom. It’s not always possible to join an activity on the same day you make your referral but we often run the listed activities on a weekly basis. |
